You may be responsible for the payment in full at the time of your appointment, if you have not provided your insurance prior to your scheduled appointment and we are unable to obtain approval of coverage and benefits for behavioral health and pre-authorization if required by your carrier. Otherwise you are responsible for your co-pay/co-insurance, if applicable.
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking your insurance carrier the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
REDUCED FEE
Reduced fee services are available on a limited basis and with prior approval.
PAYMENT
Cash, credit cards (Mastercard, Visa, American Express, and Discover) and checks are accepted for payment.
CANCELLATION POLICY
We require that you notify us at least 24 hours in advance or you will charged for a No Show/No Call Fee of $140. If you cancel less than 24 hours, on the same day of the appointment, you will be charged a Late Cancellation Fee of $90. Your insurance company may not cover or reimburse you for missed appointments.
